Job Title: Pharmacist Home Infusion / IVIG / Hemophilia
Location: Fully Remote (Preferred: TX, MO, FL)
Assignment Duration: 12 Months
Work Hours: Between 8 AM 6 PM CST (shifts within this window)

Job Summary:

We are seeking experienced and clinically knowledgeable Pharmacists to join our Pharmacy team. This role involves providing remote clinical oversight and support for home infusion therapies such as IVIG, hemophilia treatments, and other complex specialty drugs. Pharmacists will play a key role in patient counseling, data verification, prescription clarification, and collaborating with providers and nurses to ensure safe and effective care delivery.

Key Responsibilities:

  • Review and verify complex specialty prescriptions for accuracy, dosing, and clinical appropriateness.

  • Coordinate with provider offices to clarify prescriptions, infusion protocols, flush schedules, nursing needs, and infusion supply requirements.

  • Counsel patients on home infusion medications and create individualized care plans.

  • Monitor prior authorization requests and ensure compliance with plan design and formulary criteria.

  • Provide clinical support to internal departments and external providers regarding medication therapy management and infusion protocol details.

  • Support medication therapy management (MTM), including DUR reviews for Medicaid and Medicare populations.

  • Assist in developing clinical policies, procedures, and drug utilization review (DUR) protocols.

  • Document and maintain accurate patient interactions, interventions, and therapy plans using electronic systems.

  • Support ScriptMed Cloud implementation by providing clinical insights, feedback, and system best practices.

Required Qualifications:

  • Bachelor's or Doctorate (PharmD) in Pharmacy from an accredited program.

  • Active, unrestricted Pharmacist license in TX, MO, or FL (required).

  • Minimum 3+ years of experience in mail-order, hospital, infusion, or specialty pharmacy; or 1+ year of residency program.

  • Strong clinical acumen in infusion therapy, specialty medications, and rare disease treatment protocols.

  • Proficient with pharmacy software systems and clinical documentation.

  • Excellent written and verbal communication, data analysis, and customer service skills.

  • Ability to work independently, prioritize changing tasks, and maintain cohesive collaboration within a remote team.

Preferred Experience:

  • IGNS Certification (preferred but not required).

  • Experience with IVIG, hemophilia medications, and infusion pumps/supply coordination.

  • Familiarity with multi-state mail-order pharmacy workflows.

  • Background in utilization management, MTM, or specialty clinical review.
